Please be advised that your primary care provider must refer you for consultation and arrange for your records to be sent to our office prior to an appointment. Your medical records are important in the evaluation of your rheumatologic disorder.

What to bring
- Bring copies of the permanent x-rays and/or their reports for our review
- Bring all medications you are taking (or an accurate list) to each appointment
- Bring all insurance information and any necessary referrals from the insurance plan.
